Answer: Lebanon, Kansas, where the brothers lived

The bunker was in Lebanon, Kansas. In the episode, viewers got to see a bit of the small town, and see the brothers as part of the community, although under assumed names. The local bartender knew the "Campbell brothers' usual" and the postal worker was charmed by Dean, but did not trust Sam. According to the sign at the edge of town, Lebanon was the geographical center of the United States.
2. In "Lebanon," when Eliot was speculating to the other kids about the brothers, how did he refer to Castiel?

Answer: Their weird sidekick with the trench coat

Eliot said, "I mean, think about it. Where did they even come from? Them, or their weird sidekick with the trench coat? And what about that kid with the dumb Bambi look on his face all the time?" Yes, the latter comment was a reference to Jack.

When John appeared, the brothers were very happy to have their father back. Sam was finally able to get closure with his father. Dean was able to have his family together again. But they then realized that John had come from 2003, and by bringing him forward, their entire history was changing, as well.

As part of that, Mary would no longer have been brought back by Amara. When John heard that Mary would be gone if he stayed, he wanted to go back, even knowing he would die in just a few years.
5. Which two angels attacked the brothers in "Lebanon"?

Answer: Castiel and Zachariah

Zachariah and Castiel came down from heaven to find out who was "playing back to the future" and messing with time in Lebanon. When he saw the Winchesters, Zachariah told Castiel to kill them. Castiel from 2003 did not know them. He attacked Dean. Zachariah attacked Sam. Sam was able to kill Zachariah with an angel blade. (It was nice he got to do so, since Dean got to kill him in season 5.) Sam then used the blood sigil to send Castiel away before he could kill him and Dean.
6. What were the brothers hunting in "Ouroboros"?

Answer: A gorgon

After at least six deaths in Northern New Mexico, Rowena and Sam discovered that the Winchesters were hunting a gorgon, which Rowena explained was "an ancient, cursed being with an affinity for snakes and a hunger for human flesh... Legend has it every few months the gorgon goes on a spree and gorges itself." This fit with Sam discovering seventeen unexplained deaths which may have been the work of the gorgon.
7. In "Ouroboros," how was the killer able to elude the brothers for so long?

Answer: He glimpsed the future when he ate his victims' eyes

The monster kept seeing the brothers coming when he ate his victims' eyes, so Rowena's tracking spell was not getting them to scene in time to save the victim. Fortunately, the monster was unable to see angels, so Jack and Cas were able to surprise him and the four hunters then fought and killed him.
8. Who died at the end of "Ouroboros"?

Answer: Maggie

Dean was unconscious after his fight with the monster, and Michael escaped from his mind. He then possessed Rowena and went on a killing spree in the bunker, killing Maggie and the other hunters from Apocalypse World. Then, as Michael tortured the Winchesters and Castiel, Jack killed him, consuming his grace, but burning off a large chunk of his soul in the process.

Sam and Castiel went hunting after a man's head exploded in Arkansas. Charming Acres seemed to be a town stuck in the past. Castiel said it was like stepping into a "Saturday Evening Post." The town had a milkshake shop, everyone dressed like they were in the 1950's, and they were completely cut off from the rest of the world, with no modern technology.

Answer: Human psychic

Sam and Castiel thought they were hunting a witch or a demon after a man's head exploded. Then, after it happened to another, Sam was brainwashed like the rest of the town, thinking he was happy in the town. It turned out that the mayor was a psychic and he was controlling the whole town and people in it, to create his own little piece of the past.

Then, when someone starting breaking away from his psychic connection, he killed them. Sam and Castiel fought him, and his daughter tapped into her own powers to break his connection.

She trapped him inside his own mind, happy but harmless.
11. What happened to Jack's snake in "Peace of Mind"?

Answer: Jack killed it

After Jack spent much of the episode caring for the snake, and trying to find food it would eat, Jack killed the snake. He told the snake that since the snake missed his friend, he was helping it to see its friend again, by killing it, so it would go to Heaven. The episode ended with Castiel, looking concerned, watching him do that.

Answer: Kohonta

The brothers had never encountered a victim with burns around the bite marks before. They researched, and eventually Sam found the local Native American legend of the kohonta, "twisted monsters that roam the woods, driven to consume sweet mortal flesh." Sam also told Dean that, "according to the lore, the kohonta get so starving, they spit up stomach acid." The sheriff explained the kohonta was Henry Parker, who had been cursed by his tribe, "doomed to roam the woods, and if he did not eat, his body would eat itself."

While the brothers were hunting, Jack hung out with the town kids the brothers met in "Lebanon." Jack wanted the kids to like him, so he was showing off. He used his powers to move the blade with his mind. The kids were impressed, at first, but then he went too far, and scared them. When Stacy panicked and tried to run, he accidentally stabbed her with the angel blade. He cured her, but the kids were afraid of him, and left him.

After their hunt, the brothers told Jack that the reason they had not brought him along was because they did not think he was ready to use his powers. Jack promised not to use the powers without permission, but he also lied to them, not telling them about using his powers while they were gone. For a little comic relief, he also explained he had not bought beer because his ID was fake.
