This episode, the series' 150th, was a busy one - a train derailed nearby, causing a huge accident and providing many patients for the ER. Drs. John Carter and Luka Kovac are sent to the accident scene to help out, and eventually, Dr. Elizabeth Corday joins them. At the episode's end, Dr. Kovac goes to see Bishop Stewart, and is with him during his last moments and as he dies. James Cromwell played Bishop Stewart in 4 episodes, with "The Crossing" as his final appearance.

Among the episode's events were a solar eclipse, a mass suicide (by ingesting cyanide), a near-drowning, an ongoing gang war, a fire at Doc Magoo's, and the amputation surgery for Dr. Romano.

Dr. Pratt had a great shift, and decided to stay at County instead of transferring to Northwestern (a nearby hospital) as he had planned. Dr. Carter had a horrible shift, and decided to go to Africa to help out Dr. Kovac, who was already there.

Chuck, Dr. Susan Lewis' ex-husband, was supposed to be on the flight, but there was no room for him. He survived, but sustained some injuries that forced Abby and the other med students to intubate him and put in a chest tube (procedures that medical students do not normally perform).

Paramedic Zadro, a constant fixture on "ER," was in the ambulance bay at the time of the crash, and sustained a head injury.

Though Neela was on the roof when the helicopter began 'thrashing' around, she remained uninjured (though the nurse she was with was not as fortunate).

"Time of Death" was a rare episode in which the traditional opening-credit sequence was not shown. The actual credits were shown on screen while the show was airing, but the theme song, along with the shots of each character and actor that normally air, was not played.

The episode plays out in real time -- in the 44 minutes that are shown to viewers on screen, only 44 minutes of time actually passes in the "ER" world. Through the first 12 seasons of "ER," it is the only episode to be shown in this manner. [Note: The real-time aspect is also demonstrated when Ray Barnett (Shane West's character) orders pizza, and keeps track of the time, making sure that the pizza place will keep its promise of delivering with 40 minutes (or the pizza is free).]

This episode featured Ray Liotta as Charlie Metcalf, an alcoholic and ex-con who is treated by Sam, Dr. Kovac and Dr. Pratt. Liotta won an Emmy in 2005 for Outstanding Guest Actor in a Drama Series for his role in this episode.
