Luke's father sits at home and drinks the pain away. After slicing his son's throat and leaving him for dead outside of Garrett, he's clearly haunted by his decision. There's a knock at his door though. In the summer house, Zoe remembers waking up to find Joseph operating on her, removing a centipede from her mouth while she lay on an outdoor marble slab. Zoe later wanders the halls to hear the Peach Family arguing in another room about a delivery that must be made. Mrs. Peach says that they need to bring the little girl that they marked to secure mercy; Joseph doesn't seem to be worried, instead listening to Edie's pregnant belly.

Zoe continues to search the summer house until, at the end of a hallway, she sees a solitary red door at the end of a blood red carpet. Something horrible draws her towards it, and as she enters the brightly-lit white room and nears the door, the horrifying thing within scares her away.
Outside Garrett, Luke's father leads Peach's men out to the field where Robert and his son remain in the open grave. He explains that he believes in family and retribution, but when they check the pit, Luke's body is gone.
The truth is, Louise retrieved Luke from the grave and brought him back to her house. With her skills in taxidermy, she sews his throat closed and saves his life.

Alice awakens in the summer house and listens to the floors and the walls for something she can't figure out. Zoe returns to the room worried about what may be there with them, reasoning that whatever she saw must be what the Peach Family prays to. When Zoe reveals the injuries on her leg and explains her hunger, Alice says that anything is better than going insane, and she needs all the help she can get to avoid a terrible fate. After the argument, Zoe leaves to search for a door that will take them downstairs and when she leaves, Alice decides to follow the noises in the walls.

The scratches in the walls lead Alice downstairs and into the room where Joseph Peach saved her from the manifestation in her mind, and when she gets there she finds the bloodstains still on the floor. Placing her hand on one of the walls, it seems to push back towards her until something tears through.

It's Izzy. She asks Alice if she can go home, but Alice pushes her back inside; she has to wait just a little bit longer.

Luke is finally able to sit up when Louise bandages his wound though it's clear he's still in immense pain. She explains that his father missed the artery in his neck, otherwise he'd be dead. When they hear Aldous Peach outside, Louise brings Luke down to the basement and grabs a gun. In the basement, she begins to pry the boards from the bootlegger tunnels.
Alice finds Joseph Peach in the study playing a game of chess with a horrifying, skinless creature. She asks about Izzy and Peach assures her that they don't eat children. He would, however, like to find Izzy to bring her home and then he can get to curing her. Alice says that she'll show him where she is.
Alice certainly does give away Izzy's hiding spot, and before Izzy knows it, Edie is walking her down the hall to meet with the skinless man. All three of them walk through the red door.

Louise tries to work with haste as Aldous quickly finds the locked basement door and tries to break through. Aldous, in doing this, doesn't notice the gun being pointed at his head from behind. It's Luke's dad though. He finds that Louise has fixed up his son and stolen Robert's head in the bowling bag and he tries to explain that he did what he did out of panic; the Peach Family isn't the mob-- they're worse. Luke and Louise explain that Aldous and the others will only get back up, even if he shot them, and they need to remove their heads. Zoe tells Alice that the Peach family is looking for a kid, but Alice lets her know that it's okay.

They said that they plan to let her go. Soon, they're retrieved by the skinless man. It's time to go, Alice says.

Alice and Zoe are brought to meet with Joseph Peach and he is found standing at an altar with the rest of the family watching on. While Zoe sits nervously to the side, Alice is placed atop the marble altar and her head is dipped back into a cistern of blood. Joseph places a pair of pliers into her mouth and extracts the same thing he did for Zoe-- a centipede. The ceremony is declared a success though Zoe is still very worried.
In Louise's basement, Luke's father saws into Aldous Peach to decapitate him. He stares back at his son while he does so.

Zoe, now frustrated, walks through the summer house and down the hallways ready to face the red door and when she arrives she does not let herself be frightened by the presence within. Instead, she walks right up to the portal and opens it, forcing her way inside.

She emerges into blackness and finds a single light within where Izzy lays in the center of an eight-pointed star on the floor. She lifts the girl up and races away with her while a demonic presence with antlers watches from a distance. Zoe races back out the red door and through the summer house, but the place is a labyrinth.

They end up at a doorway and, sure enough, it's a staircase leading downstairs to their world-- the real world. Zoe sends Izzy through, telling her to meet at Louise's house if she doesn't return.

Zoe returns to her and her sister's room to find Alice staring into a gold tin at her centipede. Luke wonders what they do next and hears his dad talk through a story, but at the end of it, Luke shoots him in the chest.

Zoe is back downstairs in Medallion Park. She removes her shoes and runs as fast as she can into the night.
Alice informs the Peach Family that Zoe left; she was all worked up about Izzy. Joseph Peach immediately dons his hat and leaves the dining room. He checks behind the red door and finds it empty inside. That which is deeper within-- his God-- is angry. He vows he will bring back the child that they marked.
