This was established in the episode "Hope And Fear" by Seven of Nine.

2. Who was one of the inspirations that led to her joining Starfleet?

Answer: Amelia Earhart

Amelia Earhart was one of her idols and she even met her in the episode "The 37's". Da Vinci was another of her favourites, she had a holodeck program about him on Voyager (established in "Scorpion, Part I").

3. What is her sister's name?

Answer: Phoebe

Phoebe was the artist of the family. We find out a lot about her in Jeri Taylor's book "Mosaic."

4. What sport did she play in high school?

Answer: Tennis

She played tennis in high school, but afterwards didn't play for 19 years. She played again for the first tim in "Future's End, Part 1"

Answer: Drowning

He drowned in 2358. Kathryn grieved deeply after that as she was very close to him.

She and Tom Paris crossed the warp ten barrier together and evolved into amphibious creatures. In this state she and Tom mated, producing three amphibious children. They later left those children on a planet in the Delta Quadrant.

Answer: USS Rhode Island

She served as a science officer on the Al-Batani, as a commander on the USS Billings and as the captain on Voyager. She never served aboard the Rhode Island. Harry Kim, however did so, in the future in "End Game, Part 1".
