The fall of Cocoon is what this refers to, when it was spared from crashing into Gran Pulse at the end of "Final Fantasy XIII". This information can be found in the data log within the game.

This on is pretty self-explanatory. Once you solve a paradox in one time, you change the future and create an alternate reality. The letter x is what is used to separate similar time periods.
