1. In the first scene of the movie, the subtitle describes the setting of the movie. What state does the subtitle say?

Answer: Louisiana

The movie is set four years after the end of the Civil War, and the beginning scene is set in the state of Louisiana in the dead of night. A crazed man, wearing a thick metallic collar, is running away from a giant flying sawblade, muttering ravings about a plot to destroy the United States government.

But before he finds help, the blade slices his head off. A few seconds later, an old, former Confederate soldier walks up to the doctor's body, pries the sawblade out of his neck while muttering the ironic musing, "And they say you scientists are supposed to be smart."
2. Will Smith portrays the hero of the story. What is his name in the movie?

Answer: Captain James West

Will Smith plays the 'legendary' Captain James West, a government agent who has been on the trail of the Confederate general in the opening, General McGrath. He could easily be described as "bullet-enthusiastic" (to say it in a polite way).
3. Later on, in Morgan, West Virginia, West is 'entertaining' a beautiful woman in the water tower. What is the name of the actress who plays this woman?

Answer: Garcelle Beauvais

Garcelle Beauvais plays Belle, Jim West's 'date' in the Morgan, WV water tower. She has also worked in films such as "Coming to America", "Barbershop 2", and as ones recent as "I Know Who Killed Me".

While they are 'occupied', some men who work for Gen. McGrath are loading crates into a cart below, but the energetic horses back the cart into the tower, making it fall and spilling a nude West right in the midst of the men.

Fat-Can-Candy's is the saloon they are at, as read across the front of the building at the beginning of the scene (watch closely!). And it seems like Artemus's disguise is working like a charm!

Once on board, West pries open the crates (being as suspicious of McGrath as he is) to see what McGrath's men are trying to deliver. The crate is indeed full of the highly volatile chemical, being transported in a completely inappropriate manner.

"This is NOT THE WAY . . . YOU TRANSPORT . . . NITRO! WHOA!" shouts a desperate West as the cart hurtles down toward the saloon and the unsuspecting people within it.
6. The name of the physician kidnapped by McGrath is Dr. Escobar.

Answer: False

He is not a physician, he is a hydraulics expert. His name is Escobar, though. Gordon is just about to find out whose payroll McGrath is on, in exchange for the kidnapping of this man and many others, when 'she' is interrupted by Captain West. The ensuing confrontation sends the bar into bedlam.

Just as West and Gordon find out that they are after the same criminal (and West finds out that this prostitute is actually a government agent as well), the cart of nitro is released from the top of a nearby hill, and it runs directly into the bar. They say this to each other, wondering what the situation is now, on top of the chaos that has just occurred. They are a little late in realizing that the crate of nitro is hurtling straight for them. They do manage to escape the bar before the place blows to smithereens.

In truth, Ulysses S. Grant went to West Point University, NOT Harvard. Actually, West figures Gordon out from the way he waxes eloquent about himself while in character as President Grant, but whether it's that reason or another is based on opinion. Or is it?

Both Gordon and President Grant were portrayed by Kevin Kline, so it must have been easy for him to play a 'disguised' Grant and the real Grant.
