If any character dies at any point and for any reason, they're gone for the rest of your playthrough. Technically, the player does have the option to reverse the occurrence using the game's Scene Selection function, but the game will never force the player into a game over scenario if a particular character dies at any point, be it the beginning, end, or middle of the story.

Answer: Nick

Depending on player decision, Rachel can either completely break up with Eric, break up with Nick, continue her relationship with one or the other, or reject them both. This ends up being a secondary portion of the character development of each of these three.

Answer: Corporal Merwin

Merwin will either bleed out as Jason and Nick fail to tend to his wounds in time, be left behind by them and torn apart by vampires, or get shot in the head by Dar as the Iranians ambush the Americans. He cannot live through the entire game no matter what. Other characters with similar distinctions are Clarice, Dar and Joey.

Joey bleeds out amidst a shootout with Iranian troops. His fate cannot be changed. He will later be found, risen as an undead amidst a chasm underground, where he will be finished off.

Clarice will succumb to infection somehow, and will either be put out of her misery by her former allies or left to an uncertain fate as she rises as an undead. She cannot avoid this fate.

Dar will be torn apart by vampires as they bear down on the cast of characters inside the temple. His fate also cannot be changed in anyway.

Answer: Eric is killed

The player will be prompted to cut the rope and given a certain amount of time to cut it. If the time elapses, Eric will try once more to pull Rachel up and will fail. The game will once again prompt the player to cut the rope after this happens. If the time elapses once again, Eric will slide off the cliff, causing Rachel to fall, while Eric is almost immediately impaled by a large rock sticking out of a boulder, killing him instantly. No matter what, Rachel will not actually die whether Eric does or doesn't cut the rope, so if a player is trying to have all main characters survive through a play through, they must eventually cut the rope in order to not cause Eric to die in this sequence.

Answer: Eric

Eric has a total of seven different chapters in which he can die out of twenty-two playable chapters. This is one more than Rachel at six, who has two more than Salim and Nick, with four apiece.

Answer: Balathu is killed and becomes undead

Regardless of what happens, Balathu and Kurum are eventually cornered and overwhelmed by a few vampires who easily slaughter them. What ultimately happens to Kurum is unknown, but Balathu eventually rises as an undead and directly opposes the main characters of this game as they travel through the temple much, much later. Balathu is known in-game as the "Ancient Warrior" and he can be killed for good depending on player decision.

He can also potentially cause the deaths of every main character in the game as well.

Answer: Stake to the heart

Salim discovers that even a giant truck which falls on top of a vampire does little more than agitate it. However, when Salim seems to have been cornered himself and death unavoidable, a hole in the cavern's ceiling brings sunlight onto a small circle he stumbles into, which immediately incinerates and destroys the attacking vampire.

He is also able to learn, seemingly on accident, that a metal crowbar he has picked up can be jammed into the creature's heart and used to kill them that way as well.

He notices that, while guns and general military weaponry can slow the creatures down, they can never be actually killed even by very heavy machine gunnery and explosives.

Answer: An underworld where human souls are tormented by demons

The 'house' aspect is figurative- the idea is that, according to mythology, anyone who died within the temple grounds would never truly 'die' in the strictest sense.

Answer: An eclipse

Ordinarily, these vampires have proven to have an extreme sensitivity to sunlight, to the point where being under the sun will completely disintegrate them. However, at the worst possible time, once the characters finally make it back above ground, they are enshrouded in darkness as a lunar eclipse takes place.

Not having to fear the sun, a large group of vampires pursues them, and this ends up being the final fight scene of the game as the characters take shelter and prepare to fight for their lives inside a barn house.

Answer: Flares

The idea is to light up a flare, and then shove it directly into the open, gaping jaws of an incoming vampire. It actually works pretty well, too- if the Ancient Warrior is still alive at this point, he will attack the barn house, and the only way he can be killed is through shoving a flare into his mouth and causing his head to explode.

This barely allows any characters alive at this point to fend off the vampires until the sun returns to ward them away.
