1. The first puzzle you encounter involves an altar at the start of the Shrine Path. How many items must be placed here?

Answer: Five

After Hinako falls unconscious and finds herself seemingly in another world, she'll awaken to see that she's positioned before an elaborate shrine. This will ultimately be her first puzzle experience, and it will test her ability to thing laterally based on items presented.

The prompt will be the same no matter the difficulty: "Thy restless heart betrays thee as unfit for this auspicious day. Thou must first pay respect to the gods and the spirits of thine ancestors. A restless heart leads to a corrupt soul. Only through sufficient offerings laid upon many stands may thy soul be cleansed." Before Hinako will be five items: a fan, a jar, a rat carcass in a leaf, a bird in a robe, and a fish.

Placement of these items will be determined by the complexity of the puzzles based on your playthrough, but in every case you'll have to examine the items closer. In Story Mode she has to match knots on the items to their pedestals; in Hard Mode it's leaves; in Lost in the Fog Mode you'll have to solve riddles.
2. Which of these seals the Shrine Vault shut?

Answer: A combination lock

During your visit to the Shrine Path, the mysterious Fox Mask will bring you to the Shrine Vault and indicate that your way forward will involve getting inside to claim the Stone Key. Naturally, the door will be locked with a complex, three-symbol combination. The only way to get inside will be to wander around the paths here, searching the walls of Ema tiles for those matching items listed in a provided text, and these will be different in each difficulty. Flipping the tiles will give you the symbols which will then fill in in your journal.

The last of these tiles will also feature a Combination Lock Dial, necessary to complete the lock and open the door. The Stone Key inside the vault can be placed with the Inari statue at the shrine nearby.
3. To get through the rice fields on the way to Rinko's house, you'll need to remove which of these from the scarecrows along the way?

Answer: Thorns

Heading to Rinko's means crossing the farmers fields next to Ebisugaoka and this will put Hinako into an odd spot. Getting deeper into the fog it will appear that she's passed into an endless field, occasionally punctuated by pockets of scarecrows. A Mysterious Note will give you a hint though; some of these scarecrows aren't scary. One will bear a riddle that you must decipher and, then, you must choose the one of the scarecrows that fits the prompt and remove a thorn from its chest. The scarecrow will then contort to point the way to your next destination.

Some of these scarecrows, however, while shaped like schoolchildren, are actually hungry monsters, and one wrong move will set them upon you. Five correct answers will get you out of that puzzle.
4. The Puzzle Box in Ebisugaoka Middle School is found on whose desk?

Answer: Rinko's

Once you open the second floor corridor in Ebisugaoka Middle School, the first open classroom on the right is of relevance since it's the one containing Hinako and her friends' desks-- she'll even point out which is which. On Rinko's you'll find two important items, one being an unopened envelope, clearly a romantic letter to Shu, and the other being a complex puzzle box.

The key to solving this is in the three pages of a trio of secret letters; you need to reveal the symbols on the box that apply to the clues provided. Once it's fully opened, the box will reward the Second Floor Classroom Generic Key.
5. The kids of Ebisugaoka Middle School use codewords to encrypt which of these?

Answer: Locker combinations

The first floor of Ebisugaoka Middle School contains numerous student lockers, and to get out of the building, Hinako needs to make sense of at least one of the codes to open them. The answer for this puzzle can be found in the numerous folded notes and books around the school, and she'll need to piece them together in her journal to make sense.

The code itself is quite easy to master as most of the words that are encrypted, while cryptic based on the clues, are simple substitutions wherein the number in the code looks like the letter you're looking to decode. I = 1, for example, and A = 4. Asakura Ayumi's locked is the one you need to open to escape the school, and the password is different based on your difficulty.
6. Opening the door to Hinako's room in the Shimizu Household requires four plates, three of which she'll have to search for. Inside her room, she'll get an Emblem bearing which design?

Answer: Pine Tree

The Shimizu Household won't quite be the way that Hinako left it at the start of the adventure as nothing will be in its right spot and the house will have grown significantly in size, with corridors linking to looped versions of the home. Some of the rooms, including Hinako's bedroom, are still there, but slightly different in their own ways.

Hinako's room is also locked by a puzzle bearing four holes in which to place different plate designs, all of which can be determined by a poem posted outside. One of the missing plates will be in the playroom; another is in the living room; the last is in the sitting room with birds on its doorway.

Once the pieces are in place, Hinako can head inside and collect the Pine Tree Emblem, necessary for opening the door in the house that has Shu's voice on the other side. And beyond this, Hinako will face off against her parents... or whatever they've become.
7. To get the Scale Plate, Hinako needs to balance how many kokeshi dolls?

Answer: Five

One of the rooms in the Shimizu Household's many corridors is a playroom that Hinako may have had when she was much younger, and here, on a table in the middle, is a set of scales that she used to play with. The puzzle here is quite straightforward. In front of the scale is a set of five kokeshi dolls, one of which is missing its head. All you need to do is balance the two sides of the scal exactly; the headless one goes on one side while all four of the others go on the other.

When completed, the scale will reveal the Scale Plate-- needed to access Hinako's room once lal the other plates are in your possession.
8. Of these, whose portrait will be missing from the walls of the living room in the Shimizu Household?

Answer: Hinako's

In the living room of the Shimizu household, Hinako will be faced with a puzzle that involves an ominous group portrait and a number of single portraits. The largest painting will feature a number of people-- all friends and family in Hinako's life-- but one or more of these faces will be rubbed out with black marker. Your goal is to flip the portraits around the room so that anyone whose face is visible is rightside-up and anyone who's scribbled out is upside-down.

One photo, however, is missing entirely. You can find Hinako's missing portrait in a storage room in the second loop of the house.

Once all of the portraits are placed correctly, the larger, ominous painting will fall off the wall and reveal the White Bird and Fox Plate, the second missing plate needed to get into Hinako's room.
9. The large mural in the Treasure Hall of the Dark Shrine has numerous slots in which to place Crests, but how many do you actually need to use?

Answer: Three

The large mural appears to be the only thing blocking off the Treasure Hall proper, and it depicts a number of foxes in procession. Here, the puzzle involves using another provided poetic clue to place a trio of Crests into the proper slots to fulfill the prompt, and the area's mission is to navigate a labyrinth to find the missing pieces and make it work. Once you find the Black Sparrow Crest, White Sparrow Crest, and Fox Mask Crest and put them into their proper spots, the wall will vanish and grant the path to the ring ceremony, marking one of the final rituals that Hinako must undertake in the other world.
10. Per your Journal, there will be two puzzles that are only completable in New Game+. Both of these involve the collection and use of which of these, which can change the ending of the game?

Answer: A sword

Though the story will appear to end with Hinako facing the Fog Monster, a manifestation of her own trauma and violent psyche, you can restart the journey from the beginning in New Game+ (several times) to uncover more of the truth behind her nightmare, and in doing so you might find that things can change.

Two additional puzzles can be completed in these later playthroughs, and both follow the collection and cleansing of a Sacred Sword.

The first will have Hinako collecting documents and research photos leading to a series of jizo statues that will take her to the Sacred Sword, which can be found at the Divine Tree in Ebisugaoka. The second involves cleansing the sword with Crimson Water, purifying it of its curse at a basin at a shrine in Sugisato.
