Answer: Earth 3 CSA

The CSA earth was populated by evil counterparts of the Justice League. Although not as numerous as the normal roster it was made up of Ultraman (Superman), Owlman (Batman), Superwoman (Wonder Woman) Power Ring (Green Lantern) and Johnny Quick (Flash). The only hero of that universe was Alexander Luthor. Luthor and his wife put their son in a rocket capable of traversing dimensions to save him in a homage to the origin of Superman.
Earth 4 contained the characters purchased from the Charlton company including Blue Beetle, Nightshade, The Question, Peacemaker, Judo-master Captain Atom and more.
Earth X was a world where the Nazis won and only the small group known as Uncle Sam and the Freedom Fighters opposed them.

The three non answers were the last three remaining Earths along with the prime two. All five were merged and thus a majority of the super powered inhabitants survived.
Earth S was the world of characters bought from Fawcett including the Marvel family (Mary, Billy and Hoppy the Wonder Bunny among others).

Answer: Arion

Arion is a Homo Magi an evolutionary divergence who had perfected magic. He was one of the most powerful of Atlantis's magicians. He first appeared in "Warlord" 55 in March 1982. This was in Issue #1 of "Crisis on Infinite Earths"(1985).

Answer: Batman and Joker

Joker is initially freaked out by his appearance and is stopped from killing Batman. The Joker fears he is here to help Batman, but it is only a spatial after image. In the ensuing chaos the Joker escapes.

Answer: Dr Light IV

Dr Light aka Kimiyo Hoshi was a scientist who was granted light based powers by The Monitor. Her first appearance was during Issue #4 of "Crisis on Infinite Earths"(1985). Wildcat III would also appear later in the series inspired to take up the costume after the current Wildcat, Ted Grant, is crippled by falling debris.

Answer: Brainiac

This led to the realization that in order to survive the crisis he would need the help of other villains. This was the totally robotic version of Brainiac. He set course for Earth to find Lex Luthor.

Specifically they ended up in Markovia, the home country of Geo-Force. There they fought both the Nazis and the Shadow demons, minions of the Anti Monitor. They were aided by Sgt Rock and Easy Company, the Haunted Tank and The Losers. The Losers were all killed in the issue along with Farmer Boy/Flower of Easy company.
This occurred in Issue #3 of "Crisis on Infinite Earths"(1985).

Answer: Lady Quark

Lord Volta and Princess Fern were her husband and daughter who died defending their world. It was the first appearance for all three. He saves her despite her pleas to let her die with her family. He though believes that he will ease the burden on his soul by doing this small act.

Answer: Red Tornado

Although originally a being from another planet known as the Tornado Champion, the Red Tornado was revealed as an air elemental. It was never fully explained what the Anti Monitor's plans were for Red but he ended up being sent back and was nearly destroyed.
Both Swamp Thing and Major Disaster were earth elementals. Firestorm was a fire elemental.

Answer: Krona

Krona was an Oan who despite repeated pleas from his fellow men he dared to create a device to view the origin of the universe. His machine exploded and caused a reformation of the universe into two parts. One the positive matter multi-verse and the other the anti-matter universe.

The creation of the anti-matter universe caused a sister planet to Oa known as Qward and on this planet the Anti-Monitor was born. Up until this was revealed by Harbinger in Issue #7 of "Crisis on Infinite Earths"(1985), Pariah had believed he had been behind the Anti-Monitor's creation, although he did reawaken him from dormancy.

Answer: Supergirl

After seeing her cousin the Superman of Earth-1 near death, she takes the fight to the Anti-Monitor. She destroys both his machines and his psychical body but in a last attack he lashes out and delivers a fatal blow. The issue ends with a memorial service being held for her. Superman says he will miss her forever but his grief was not to be.

By the end of the series was over and time was rewritten Supergirl was one of the characters who was excised from existence and thus almost no one even knew she existed.

In order to achieve enough speed he had to go faster than he ever had before. He knew that doing that would result in him merging with the speed-force itself and for all intents and purposes kill him. His last thoughts were of his girlfriend Iris, his protege Wally West and his closest friends. Unlike Supergirl's death he was remembered post crisis.

This happened in Issue #8 of "Crisis on Infinite Earths"(1985).

Psimon struck a blow with his mental powers that he thought destroyed Brainiac. He was about to kill Luthor as well when Brainiac revealed that he was not that easily killed and slew Psimon. Meanwhile on the Earths the heroes and villains fought casualties were had on both sides. Chemo poisoned the New York harbor and in the process killed Aquagirl, while Speedy destroyed the monster known as the Shaggy Man with an explosive arrow.

The battle came to an end when the Spectre appeared and told them that their true foe still lived and had fled to the beginning of time to resume his quest to destroy the multi-verse.

This happened between Issues #9 and #10 of "Crisis on Infinite Earths"(1985).

The two pronged result send the villains to Oa to try and stop Krona from enacting his experiment which created the Anti-Monitor to begin with. Most acted as decoys and were soundly defeated but a small group consisting of The Icicle, Mirror Master and Maaldor the Dark Lord made it to Krona's lab only to be utterly obliterated. The heroes were more successful in their battle at the beginning of time but only due to the intervention of the Spectre.

After the battle it appeared that everything had been returned to normal but it was not so. The Superman of Earth 2 was the first notice something was wrong. He walked to his job as editor of the Daily Star. He walks in and sits down at what he thinks is his office but is yelled at by Perry White editor of the Daily Planet. He then journeys along with the Superman of Earth 1 to Jay Garrick, the Flash of Earth 2 who has the ability to pass between Earths. When they along with Kid Flash try and travel they find that there is nowhere else to go. When history reset the history of Earth 1 became the template with additions being made to account for the new additions from Earths 2,4,S and X. Both Robin and Huntress found out that their history had been erased and would continue to fight nobly and lay down their lives alongside the Teen Titan Kole, to save civilians during the next and last battle with the Anti-Monitor.

The battle itself was fought on two fronts. On Earth the Anti-Monitor's henchmen the Shadow Demons menaced Earth's populace. In that battle a number of characters were cut down including Dove, Lori Lemaris, Green Arrow of Earth-2, Robin of Earth-2, Huntress, the Ten-Eyed Man, Clayface, the Bug-Eyed Bandit, Prince Ra-Man, Kole and Sunburst. Many of the characters chosen to die were at the request of series creator Marv Wolfman as he felt they were either too silly to live or redundant.
The battle against the Anti-Monitor itself involved a number of false finishes. He was finally put down after the an assault from the most powerful beings of Earth, an assist form the malevolent Darkseid and finally a final physical blow from the Superman of Earth-2 put him down.
After the battle Alexander Luthor offers the other 2 an opportunity to go to a place where they wont feel out of place. He also reveals to Superman that he has brought his Lois along too and that the 4 of them can be happy together forever.
They were not to be seen again till the 2006 series "Infinite Crisis".
This took place in Issue #12 of "Crisis on Infinite Earths"(1986).

Although occasionally a select few did remember bits and pieces, Psycho Pirate was the only human to remember everything. This knowledge drove him completely mad and he was locked up in an asylum. This fact was used during Grant Morrison's time on the series "Animal Man". That storyline featured tons of characters who had been wiped out during the Crisis.
