1. What is different about the opening titles of "Death in Heaven"?

Answer: Jenna Coleman has gained prominence over Peter Capaldi

Just before the opening title sequence begins, we see Clara emerging from behind a desk as the Cyberman in the room senses her presence. She then tries to convince it not to kill her by pretending that she is the Doctor. The titles then roll with Jenna Coleman's name appearing first, followed by Peter Capaldi's and then a glimpse of her eyes rather than his, effectively breaking the fourth wall and suggesting that, for this brief moment, outside the events of the episode, Clara is the lead character.
2. How many Cybermen were hidden in St Paul's Cathedral and sent off to "areas of significant population density in the British Isles"?

Answer: 91

With a group of Cybermen standing in a plaza outside St Paul's, a crowd of people gather around them to have their pictures taken with them, as if they were some sort of novelty. Suddenly, Osgood approaches and gives a signal, revealing the crowd to be members of UNIT. Kate Lethbridge-Stewart then arrives to deal with the situation, before the dome at the top of St. Paul's opens and Cybermen fly out of it. Missy says there's 91 of them and Osgood deduces that there are "91 areas of significant population density in the British Isles".

The Doctor goes further and suggests that it's "one Cyberman for each city and major town".
3. What atmospheric phenomenon do the Cybermen use to spread their "Cyber-pollen" and raise the dead as new Cybermen?

Answer: Rain

The Cybermen suddenly blast into the air thanks to propulsion in their feet, a new upgrade not seen on previous Cybermen. They fly off in different directions and the one that remains above St Paul's explodes to create a dark, rapidly expanding cloud. Soon after, the scene shifts to a graveyard where the cloud has spread, and rain only appears to fall inside it.

As the rain water fills up the drains, it makes its way towards a funeral home, where a reincarnated Cyber-Danny awakens from death.
4. What title is the Doctor given aboard the UNIT aeroplane?

Answer: President of Earth

Kate informs the Doctor that in a situation of global invasion, a representative of the planet is appointed immediately with "complete authority over every nation state". To his surprise, the Doctor was the "only practical candidate" for the position of President of Earth.

This leaves him in charge of every army in the world and the man solely responsible for dealing with the threat of Missy and the Cybermen.
5. What does Missy whisper to Osgood while she's detained in the plane?

Answer: "I'm going to kill you in a minute."

Missy gets Osgood's attention and tries to get her over to tell her "something really important" that she has to whisper. Understandably apprehensive, Osgood tries to get it out of her whilst staying a safe distance away. Ultimately, though, her curiosity gets the better of her, particular when Missy says "the Doctor will be really impressed if you learn my secrets". Osgood then leans in and hears a most unsettling sentence: "I'm going to kill you in a minute."

Missy then starts counting down to unnerve Osgood before she finds Missy's handcuffs in her coat pocket. Then, Missy lurches forward, whispers "say something nice" in Osgood's ear, and incinerates her.
6. What is Missy's "birthday present" to the Doctor?

Answer: Giving him control of all the Cybermen

Missy arrives in the graveyard and gives the Doctor a demonstration of how she can control the Cybermen with a special bracelet. She then takes it off and attaches it to the Doctor's wrist, saying, "Happy birthday, Mr President". Shocked at the thought of having an entire Cyber-army at his disposal, the Doctor argues that "nobody can have that power". Missy dismisses it and says the only way for the Doctor to stop the Cybermen converting all the humans into their own is to take them and "conquer the universe" with them.
7. Who ends up killing Missy?

Answer: Cyber-Brigadier

Missy gives Doctor the supposed coordinates of Gallifrey (10-0-11-00 : 02, its original location). Clara then asks the Doctor if he remembered those coordinates before aiming Missy's device at her, having picked it up earlier. The Doctor then pleads with Clara to give him the device so that she's not the one to kill Missy.

She obliges and the Doctor takes aim. Just as he's about to activate it, a blue shot comes out of nowhere and vaporises Missy. The Doctor and Clara turn to see a Cyberman lower its arm and point towards the unconscious body of Kate Stewart, who seemingly survived falling out of the plane earlier.

It gives a salute and the Doctor realises it's the Cyber version of Kate's father, Brigadier Lethbridge-Stewart.
8. When the Doctor goes to the galactic coordinates that Missy gave him, what does he find?

Answer: Absolutely nothing at all

Swayed by his overwhelming curiosity to see if Gallifrey had been found again, the Doctor pilots the TARDIS to the location given to him by Missy earlier. Unfortunately, when he opens the TARDIS doors, there's nothing. Missy had lied to him, possibly as one last attempt to trick and anger the Doctor.

Despite this, the Doctor lies to Clara about having found Gallifrey when he meets up with her (and mistakenly thinks that Danny found a way back to life, that they are back in a relationship and that she doesn't want to travel with the Doctor any longer).
9. What habit does the Doctor break when Clara suggests it in the cafe?

Answer: Not giving hugs

With the pair lying to each other about their futures, Clara suggests the Doctor breaks his habit of not hugging people in his new incarnation as they are about to part ways. The Doctor agrees by saying, "Why not?" before adding, "Within reason." Clara asks why he doesn't like hugging. The Doctor says, "Never trust a hug. It's just a way to hide your face."
10. Who interrupts the Doctor while he's resting and enters the TARDIS at the very end of the episode?

Answer: Santa Claus

After saying goodbye to Clara and leaving, the credits start to roll before a knock at the door interrupts them and the scene shifts back to the TARDIS, with the Doctor seemingly asleep. The knock disturbs him, and a voice calls out, telling him, "You know it can't end like that. We need to get this sorted... quickly." The mysterious stranger then declares he is coming in and, as the doors open, a gust of snow blows over the Doctor.

The stranger is revealed to be none other than Santa Claus (and played by Nick Frost, of all people) and he asks the Doctor what he wants for Christmas.

This short scene acts as a teaser for the following episode, the 2014 Christmas special "Last Christmas".
