Dr. Cox usually has a new girl's name for J.D. each episode. As this episode is an homage to "The Wizard of Oz", it is only right that J.D. is named after its primary character.

At the beginning of the episode, J.D. is relaxing in his bathtub and listening to Toto's Africa" on his iPad. When he has to go in to work, Toto, like Dorothy's dog, travels with him.

The janitor is spray-painting directional lines onto the floors of the hospital and could not pass up his 100th chance of getting back at J.D.

Dorothy's slippers in "The Wizard of Oz" are also red, which allow her to get back home. The shoes were originally silver in the novel, but were changed to suit the technicolor technology for the movie.

The song is sung as the characters are leaving the hospital along the freshly painted directional lines on the floor. Ted's band also sings the song "Maniac" from the movie "Flashdance" in this episode.

"Optimistic Voices" is more commonly known as "Out of the Woods" although this has never been an official title for the track. It is also used in "The Sopranos" during a dream sequence of Dr. Melfi.

In the movie the Lion is looking for courage, the Tinman is looking for a heart, the Scarecrow is looking for a brain and Dorothy is looking for a way home, just as J.D. tells the audience he is looking for in "My Way Home".

Carla is looking for courage to raise her child, Elliot is looking (figuratively) for "brains" (meaning intelligence) to be a good doctor, and Turk is looking for a donor heart for a patient.

Between her and Turk, Carla always believed she would be the brave parent. She starts to discover this might not be the case but has a hard time accepting it.

Turk spends most of the episode trying to find a heart donor for one of his patients. He ends up convincing Ray Bolger's family to take him off life-support and donate his heart.

Jordan is locked in her office. When the temperature rises, her recent botox-implants make her face swell and resemble the witch from "The Wizard of Oz" when she is finally killed by Dorothy.

There is a scene referencing this change, though. As Ted's band sings "We're Off to See the Wizard" and the characters are travelling along the painted lines on the floor, the color changes from normal color to a very vivid saturation, not from black and white to color.

The Janitor plays down this bizarre act by first claiming he is playing around on the outside ledge of the hospital (several floors up) and afterwards reveals "He is Green!" This is most likely a reference to the population of Oz and their rather green disposition. It could also be a reference to the Wicked Witch or her munchkins.

He talks about the animals after visiting a zoo and then notices a sexy cheerleader which makes him say "oh my". This line is taken directly from the movie where the four main characters chant this line as they travel through the forest.

This is a reference to the wizard himself when he attempts to hide his true identity from Dorothy and her friends. He, as well as Cox, fails to do so.

This is sung at the end of the episode with the camera zooming back and forth between windows of the hospital to show the conclusions to this episode's storylines. Turk is hugging the heart donor's father (having found his heart), Elliot is giving a lecture (having found her brains), Carla is playing with Jack (having found her courage) and J.D has finally found his way home.

This is one of the more famous adaptations of the song, thanks to its peculiar musical arrangement. Big Iz', the artist's, full name is Israel Kamakawiwo'ole. He died at age 38 in 1997 from complications due to obesity.
