We see Chakotay give Janeway a back/neck rub in "Resolutions" where he shares this little tidbit.

2. Who was never a grandparent on the show in any respect?

Answer: Neelix

Neelix was Naomi Wildman's godfather, but we never actually have any reference to him becoming a grandfather. We see Tom and Kes as a married couple with a grandson in the episode "Before and After". Tuvok learns he's a grandfather in a letter from his son sent via the datastream from Earth. And Samantha Wildman is a grandmother in the last episode of the series when we meet Sabrina, Naomi's daughter.

3. What is Chakotay's favorite soup?

Answer: Mushroom

Because Chakotay doesn't eat meat we know it's not ham and bean. But Chakotay and Seska share some mushroom soup in the first season and Seska reveals it to be his favorite.
