1. What item do we need to complete the Ice Ruins dungeon?

Answer: Fire Rod

The Fire Rod is needed to melt a lot of ice in the Ice Ruins. It can be purchased from Ravio for a whopping 1200 rupees! The Ice Rod and Fire Rod are each more expensive than the other items you can purchase from Ravio by 400 rupees.

The Ice Rod is needed to complete Turtle Rock.
The Sand Rod is needed to complete the Desert Palace.
The Tornado Rod is needed to complete the House of Gales.
2. Which sage do we meet at the entrance of the Eastern Palace, who insists that he is fated to be the hero?

Answer: Osfala

Osfala went in ahead of Link to the Eastern Palace, but was ultimately bested by Yuga and turned into a painting. We rescue him later on in the Thieves' Hideout in Lorule, at which time he recognizes that his fate is "merely" to be a sage.

Ravio perhaps ought to have been a hero himself, as he is later shown to be Lorule's equivalent to Hyrule's Link. It is clear he lacks courage, as he escaped from Lorule only to lounge around in Link's house while Link is left to restore order to both worlds.
Sahasrahla lives with his wife in Kakariko Village, and is identified as a very wise individual. He directs Link to the Eastern Palace, and mentions that Osfala was headed there himself.
Dampe is, as always, the gravekeeper. He works in the graveyard near the Sanctuary.
3. How do we obtain the Tempered Sword?

Answer: Upgrade it from the Master Sword by the Blacksmith

If Link finds two pieces of Master Ore, he can take them to the Blacksmith (Gulley's dad) along with the Master Sword, who will forge the Master Sword into the new and improved Tempered Sword. Then, if Link finds another two pieces of Master Ore, he can take them and the Tempered Sword to the Blacksmith in Lorule to get the ultimate Golden Sword.

Mother Maiamai can indeed upgrade many of Link's items, but the sword is not one of them.
Gorons do not forge swords in "A Link Between Worlds." However, Medigoron was able to forge the Giant's Knife in "Ocarina of Time" (N64) for 200 rupees. After 8 strikes with the Giant's Knife, or simply hitting a wall, the blade would break. Biggoron was able to forge the Biggoron's Sword, which was indestructible.
The Tower of Hera only yields the Pendant of Power as a prize, since most of the dungeon weapons (including the Hammer) can be rented from Ravio very early in the game.
4. Which of the following items do we get upgraded by completing the Advanced level of the Treacherous Tower?

Answer: Lantern

Completing the Advanced level of the Treacherous Tower in Lorule will upgrade Link's Lantern to the Super Lantern. Completing it a second time will upgrade the Net to the Super Net.

The upgrade to the Power Glove is the Titan's Mitt, which is the prize of the Desert Palace dungeon.
The Hint Glasses cannot be upgraded.
Zora's Flippers cannot be upgraded in "A Link Between Worlds," but they can be upgraded to the Mermaid Suit in "Oracle of Ages" (Game Boy Advance).
5. What character inside the Milk Bar refuses to talk to Link?

Answer: Flute Boy

Flute Boy refuses to talk to Link-- or anyone, for that matter. His partner Bard explains that he's shy and will not speak, then suggests that he and Flute Boy would be obliged to play a song for Link if he can pitch them some rupees. All of the songs that the duo play are either from "A Link Between Worlds" or other "Legend of Zelda" games.

Bard is Flute Boy's music partner, and speaks to Link on behalf of them both.
Hyrule Soldiers in the bar will talk to Link, as they explain that they fled Hyrule Castle in fear of their lives.
Gramps isn't located in the bar, but rather outside on the west side of Kakariko Village. He teaches Link the ins-and-outs of Street Pass.
6. Which mini-game is available to be played in both Hyrule and Lorule?

Answer: Rupee Rush

Rupee Rush is a mini-game located south of Kakariko Village in Hyrule, and south of Thieves' Town in Lorule. Hyrule's version costs 50 rupees per play, whereas Lorule's costs 100 rupees, and both versions only give you 30 seconds to play. The timer is hidden and Link can receive a score multiplier by returning to the entrance as close to 30 seconds as possible. Reaching 100 rupees total in both versions results in a bonus prize of a Piece of Heart.

Cucco Dash is only available in Hyrule. The object is to avoid the Cuccos flying about, and if completed on Rooster difficulty, Link will win a Piece of Heart.
Octoball Derby is only available in Lorule, with the object being to hit an Octorok's... rock... back towards the field and smash pots or hit birds. If Link scores 100 points or more, he will earn a Piece of Heart. Easier said than done.
Rollgoal was a mini-game featured in Hena's Fishing Hole in "Twilight Princess" (Wii).
7. Who tells Link about the stolen Smooth Gem? The Zora _____________.

Answer: Underling

After the Shady Guy from Kakariko Village stole the Smooth Gem from Queen Oren of the Zoras, she became hysterical and began growing in size. The Zora underling explained to Link that if the Smooth Gem is not recovered and brought back to the Queen, she may die. Upon performing this favor for the Zoras, Queen Oren gives Link the Zora's Flippers. Later we find out that Queen Oren is one of the sages that was captured by Yuga.

There is no Zora Prince in "A Link Between Worlds," but there is in "Twilight Princess" (Wii) with Prince Ralis.
There is also no Zora Spirit in "A Link Between Worlds," but there is the Lanayru Spirit in "Twilight Princess." Also, the fallen sage Laruto of the Earth Temple in "Wind Waker" could be interpreted as a Zora spirit.
8. What is the name of the witch that flies around and gives free rides to Link if he rings her bell?

Answer: Irene

Irene provides Link this service because her fortune teller told her that she must help "Green." Not knowing what to make of the fortune, she assumes Link is the "Green" that is in need of her assistance. She is later captured by Yuga and turned into a portrait. Link can rescue her from the Desert Palace dungeon, when we discover that she is a sage.

Impa is Zelda's guardian and also a sage, but she is not a witch.
Maple is the name of the witch in eastern Hyrule that brews various potions for Link.
Syrup played a similar role to that of Irene's in "Oracle of Ages/Seasons" (Game Boy Advance), where she flew around on her broom or vacuum cleaner and occasionally bumped into Link. She never gave Link any rides, but she let him keep any items she dropped if he could nab them faster than she.
9. What is the name of Lorule's equivalent to Kakariko Village?

Answer: Thieves' Town

Thieves' Town is Lorule's equivalent to Kakariko Village in "A Link Between Worlds." This same area was called the Village of Outcasts in "A Link to the Past" (SNES). Inside Thieves' Town is Thieves' Hideout, a dungeon right beside the house where Osfala's portrait is being held.

Mabe Village is the first location that Link visits on Koholint Island in "Link's Awakening" (Gameboy Advance).
Hidden Village is where Link goes in "Twilight Princess" (Wii) to meet Impaz, who saved Ilia from monsters.
Whittleton is a village of lumberjacks that live in the Forest Realm in "Spirit Tracks" (DS).

Using it will damage all nearby enemies, making it very valuable in dire situations! However, the Foul Fruit has no effect against bosses.

Using the Foul Fruit at all is entirely optional; you can complete the game without ever having to resort to it.
