In Hampton Court, England, in the year 1555, the Queen is heard crying in pain in her chambers while her servants listen at the door, worried that her decision to give birth alone in her room may have been a poor choice. When her sister, Elizabeth, arrives, she lets herself into the bedroom to investigate and congratulate on the occasion of the first-born. Elizabeth asks if she may see the newborn, but the blood on the floor and the screams of the Queen give her cause for concern.

She asks if it shall be a boy, an heir to the throne; Elizabeth claims it is a monster.

As Elizabeth searches the room for the source of whispers, two figures clad in dark robes and ornate, feathered headdresses emerge from the shadows. They take the clawed, awful creature from Queen Mary's hands, saying that he's waited more than six thousand years to be born, and all it took was Mary sacrificing her first-born for the most fruitful reign in the nation's history. The women tell Elizabeth that she, too, will make a decision not unlike her sister, but when she retaliates, they instead declare she will be barren instead.

As Anna peers over the edge of the balcony, staring down at the rotting corpse of a raccoon, Dexter breaks her out of her reverie and asks what she's doing. She also says that she and Kamal saw Preecher earlier in the evening, but he reminds her that they were told she's nothing more than a conspiracy theorist who stays around Dr. Hill's office. Anna doesn't recall him being there when the secretary told her that.

Anna says she wants to go back to Dr. Hill's the next day just to make sure. He agrees; they can go after her meeting with Siobhan.

When she heads to bed, she asks Dexter if he heard a noise in the basement earlier. He sidesteps answering, saying he just wants a normal night with his wife.

It comes together when Siobhan introduces her to Ashley and Ashleigh, the top crisis PR experts in the country. They've been called in to help rehabilitate Anna's image, and they're the best people to do it. They have a plan too-- Anna needs to make an Instagram Reel. All she needs to do is take their lead and they'l publish a feminist anthem for her. It'll work wonders. They'll break the internet with it. Anna says she'll think about it.

Heading to the bedroom, Anna is shocked to find her mother-in-law in the adjoining bathroom giving herself a facial treatment, having been let in by the doorman. Virginia claims she's only there to help, but she asks that she have a few moments to speak to Dexter alone. Virginia only wants to make sure that Dexter doesn't turn out like she and his father did; she claims she sees Anna breaking a bit. Of course, she doesn't know about the miscarriage.

Virginia's true motive is revealed quickly; she's suing Dexter's father and she needs him to testify on her behalf.

As Anna sits on her bed and combs through her hair with her hands, she notices that large chunks seem to be falling out of her scalp. Walking to her still-damaged mirror, she puts her foot through it.

At Dr. Hill's, Anna receives a new ultrasound and the doctor asks all his necessary questions to help Anna get to the bottom of the concern. Dexter and Anna are both relieved by the news, but there's the obvious concern about the past several weeks. Dr. Hill explains that this might be a case of Vanishing Twin Syndrome, where a woman is pregnant with more than one embryo, one of which is subsequently expelled. Both Anna and Dexter wonder why the notion of twins never came up before, but Dr. Hill suggests it's hard to tell these things before twelve weeks into the pregnancy. They set up a next appointment regardless.

Returning to Kamal's vehicle in the parking garage, Anna proceeds to the van only to see Ivy watching from deeper in the basement. Anna points in the direction she saw the woman, but she vanishes before Dexter can race over. They head out for dinner to celebrate instead of dwelling on what may not have been there.

In the coming days, Anna ends up filming the Instagram Reel she initially balked at, and she leaves it to Siobhan and the Ashleys to make it a success. What results is an alleged anthem for female empowerment that glosses over her obvious problem at the Gotham Awards and encourages viewers to vomit if they need to-- it's their body, and vomiting expels toxins.

The black-and-white video, urging women to support bodily functions and positivity, becomes a viral hit. Anna announces her pregnancy and shifts the narrative. And hey, even Taylor Swift reposts it.

Anna and Dexter are brought back to Talia's once their time in New York is through. Dexter immediately gets a call from Sonia, so Anna keeps herself busy until she's quickly frightened by Nicolette, there to hand over a bouquet of blowers delivered for the expectant mother. Nicolette praises Anna for her Reel; it spoke to her. She congratulates her one more time before departing, welcoming her to what she believes is a sisterhood.

As Anna checks the flowers, she finds a notecard tucked amongst the bouquet. It reads: 'You can't trust any of them.'

Anna has trouble sleeping the night she and Dexter return to Talia's and she gets up to check what's in the fridge, finding ice cream and pickles. She grabs a spoon from the drawer, finding the photograph of Dexter and Adeline still inside, and imagine's Sonia's face there instead. Taking a seat on the floor, she doesn't realize she's eaten everything without thought.

Anna heads to the basement shortly after, descending the stairs to the sound of chanting. She tries to open the stone door in the wall but finds it locked, and when she pulls harder, she slips, knocking her head on the furniture behind her.

Anna comes to in the morning, and as light streams in through the basement windows, one beam strikes the bassinet. She looks inside to find the still-decaying raccoon moments before blood drips from her mouth. Her phone buzzes as well; the Golden Globe nominees have been announced, and she's amongst them. As her name fades into her subconscious, she lifts the raccoon from its bed and raises it to her mouth, taking a bite from its corpse.
