Three of the first five Super Bowls were held in Miami (in the Orange Bowl). Super Bowl II and III were played back-to-back in the Orange Bowl, and then they returned to the Orange Bowl for Jim O'Brien's winning kick over Dallas in Super Bowl V.
2. Who of these four teams had made the AFC or NFC Championship game in the Super Bowl era, but never played in a Super Bowl in its first 40 years?

Answer: Cleveland Browns

The Browns actually made it to three out of four AFC Championships in the 1980s, losing all three to the Denver Broncos. There was the "Drive", the "Fumble", and one blow out. In the pre-Super Bowl era, the Cardinals have won the NFL Championship before. The Texans are relatively new and the Saints have only won one playoff game only to fall to Minnesota in the divisional playoffs.
3. Which NFL team has played in the AFC playoffs as well as the NFC playoffs between the merger and Super Bowl XL?

Answer: Seattle Seahawks

Seattle actually began play in the NFC but were moved to the AFC West in 1977. The got all the way to the championship game in the 1983 season and lost to Oakland. They beat Oakland in the AFC Wildcard game the next year and then lost to Miami. They were moved to the NFC West in 2002. They have had a few playoff games in the NFC and finally broke through with a win over the Redskins in 2005. That run ended in in Super Bowl XL with a 21-10 loss to the Steelers.
The Browns and Steelers were sent to the AFC along with the Baltimore Colts after the merger. The Colts actually played in a Super Bowl for the NFL (a loss to the Jets) and won a Super Bowl for the AFC over Dallas.
The Buccaneers have also switched conferences but have only played playoff games for the NFC.

Answer: Dallas Cowboys

The Cowboys have played in 8 Super Bowls, winning 5 of them. That was tied for Super Bowl victories for San Francisco but the 49ers are undefeated, meaning they have been there only 5 times. Miami had been there 5 times as well with a 2-3 record. The Steelers entered Super Bowl XL for the sixth appearance.
5. Who was not a Super Bowl MVP in the first 40 Super Bowls?

Answer: Timmy Smith

Howard was the first kick returner that has won this distinction. Jake Scott was the MVP of the Dolphins in their undefeated season. Howley was the MVP of Super Bowl V to Dallas in a losing effort. Timmy Smith rushed for 204 yards in Super Bowl XXII but lost MVP honors to quarterback, Doug Williams.

It wasn't a "gimme", but Norwood was a consistent kicker and just missed under pressure. The Bills were one of the best teams to never win a Super Bowl in the first 40, for getting as close as they did. Minnesota and Denver, before Elway's last two years, were also 0-4 but they did not lose them in a row.

Answer: Oakland

The Patriots surprised the Rams in Super Bowl XXVI but did not return the following year. The Oakland Raiders went up against the Tampa Bay Buccaneers and were pounded, 48-21. The Patriots came back and won the next two years, however defeating Carolina and Philadelphia.

Super Bowl IV between Kansas City and Minnesota had Joe Kaap at the helm for the Vikings. They lost to the Chiefs 23-7. Tarkenton did play in and lose three Super Bowls.

Super Bowl XII was a 27-10 victory for the Cowboys over the Broncos in the first game played in a dome. New Orleans had been the host city before that, but this was the first time it was indoors.

Steve Young was a hall of famer sitting behind a hall of famer in San Francisco. He watched Montana win four Super Bowls and finally got the monkey off his back in Super Bowl XXIX throwing 6 TDs over San Diego in a 49-28 win.

The Broncos won Super Bowl XXXII as a wildcard over Green Bay by the score of 31-24. The Titans made it to the Super Bowl as a wildcard, but lost to the Rams 23-16, coming up one yard short.

The Jets won Super Bowl III over Baltimore and the New York Giants have won two Super Bowls, 39-20 over Denver in Super Bowl XXI and 20-19 over Buffalo in Super Bowl XXV. However, there is another city that has won with multiple franchises. The Baltimore Colts won Super Bowl V over Dallas 16-13. The Baltimore Ravens won it in Super Bowl XXV over the NY Giants, 34-7.
Those were two different eras but they still constitute victories with two different teams.
Los Angeles did win with the Raiders, 38-9 over Washington but lost with the LA Rams in their one appearance, a 31-19 loss to the Pittsburgh Steelers in Super Bowl XIV. The Rams reached two other Super Bowls, but with St. Louis.
13. What determined which team was the "home team" in the Super Bowl?

Answer: It switched back and forth every year

Super Bowl I was considered an NFL home game year and then Super Bowl two was an AFL home game. After the merger in 1970, it switches back and forth every year. The game is supposed to be at a neutral site but if a team a Super Bowl announced for their city in advance and they got to the Super Bowl that year, they could play at home.

It has yet to happen. The only thing that it really does is give the "home team" the choice of uniform color.
14. How many points in the first 40 years was the most scored in a Super Bowl?

Answer: 55

In Super Bowl XXIV, San Francisco beat down the Denver Broncos, 55-10. The Cowboys won Super Bowl XXVII over the Buffalo Bills, 52-17 and could have had 59 if Leon Lett did not try to "hot-dog" as he was about to cross the goal line on a fumble return, Don Beebe stripped him of the ball for a touchback.

Richard Dent earned the honors after a 46-10 beatdown of the Patriots in Super Bowl XX. Dexter Jackson earned it after Super Bowl XXXVII, in a 48-21 dismissal of the Raiders. Jay Schroeder was a backup to Willams in the 1987 strike shortened season. Billy Kilmer also got to a Super Bowl with the Redkins in Super Bowl VII, but he did not win as they lost to the undefeated Dolphins, 14-7.

The Steelers were 4-0 through the seventies until 1995 where they lost to the Dallas Cowboys in Super Bowl XXX in Arizona. They did win their 5th over Seattle 21-10 in Super Bowl XL in Detroit making them 5-1. The 49ers deserve the most credit here as they are 5-0 in Super Bowls.

The Jets and Bears are one hit wonders as they have won Super Bowls III and XX respectively but have not had a return engagement.
18. Matt Millen, the GM of the Detroit Lions, won 4 Super Bowl rings as a player with three different teams.

Answer: True

He won with Oakland in Super Bowl XV over Philadelphia, 27-10 and 38-9 over the Redskins in Super Bowl XXVIII. He won with the 49ers in Super Bowl XXIV, 55-10 over Denver and with the Redskins in Super Bowl XXVI, 37-23 over Buffalo. The same success cannot be said to be repeating itself as a general manager for the Detroit Lions to date.

There is a player with five Super Bowl rings which would be Charles Haley (two with San Francisco and three with Dallas).

There are more than I listed here. The Giants scored their only points on a kickoff return by Ron Dixon in a 34-7 loss in Super Bowl XXXV. The Dolphins got a return for a TD in Super Bowl XVII but they lost to the Redskins. The Bengals enjoyed a kickoff return in Super Bowl XXXIII but still came up short to the 49ers, 20-16.
20. Which two teams have played in the most Super Bowls against each other in the first 40 Super Bowls?

Answer: Cowboys and Steelers

All of the following have seen each other at least twice in Super Bowls. The Bengals lost Super Bowl XVI to the 49ers, 26-21 and again in Super Bowl XXIII, 20-16. The Cowboys beat the Bills two years in a row, 52-17 in Super Bowl XXVII and 30-13 in Super Bowl XXVIII.

The Dolphins and Redskins split their meetings. The Dolphins finished an undefeated season with a 14-7 win over Washington in Super Bowl XII and lost the rematch ten years later, 27-17 in Super Bowl XVII. However, the Cowboys and Steelers have met three times: a Steeler win in Super Bowl X, 21-17 and a 35-31 win in Super Bowl XIII. Dallas finally won over Pittsburgh in Super Bowl XXX, 27-17.
