Ben returns to the Apollyon after his eventful first night in the evening shift with Mr. Nas and he tries to get answers out of Constance who, defensively, reminds that he never referred to their shared patient as a vegetable-- he has Lewy body dementia. He's perfectly able to move, he just so rarely does. Their job is just to make the man comfortable, and that's it.

In the other room, Mr. Phibes continues his work. Constance has Ben sit with a plate of cookies while she finishes her shift. He doesn't count the points on them, but each cookie bears thirteen.

Ben eavesdrops on the conversation taking place in the other room and overhears that Mr. Nas' bodily markers point to him having gained two years almost overnight. Mr. Phibes and Constance both seem thrilled by this news, as unreal as it may seem.

Once Constance's shift is over, Ben wheels Mr. Nas through the apartment to his nightly bath and places him in the tub, preparing for another awkward sponging in murky water. In addition to a pair of small, bony protrusions emerging from the top of his skull, Mr. Nas' feet appear to have been replaced by cloven hooves beneath the surface, if only for a few moments. A quick check of his AI search links "The Last Supper" on the sitting room wall to the number (specifically the Judas figure); the sunflowers in a vase nearby connect to thirteen via the Fibonacci Sequence; the segments on a turtle's shell in the hall number thirteen.

It all leads to another panic attack as Ben's triskaidekaphobia flares up again. He takes out his inhaler while he eyes a sketch of the devil on another wall.

The tall clown lets himself in despite the instructor's insistence that they do not accept late arrivals. When there's a continued refusal to allow him to join, he takes a pile of knives from his bag and kills everyone in the room. All thirteen of them.

But it appears to be a dream. Ben awakens from the nightmare as the clock on his laptop rolls over to 1:13:13.

In the middle of the night, Ben makes his way through the apartment following the sound of Mr. Nas' sounds of discomfort, and he finds the man standing next to the window in his sitting room. Ben approaches from behind as Mr. Nas turns to face him, showing that the old man has been vomiting a copious amount blood.

When Constance arrives in the morning, Ben expresses his concern about the black blood, but Constance chalks it up to an old ulcer that never completely healed.

All of her children were growing up by that point, so she took matters into her own hands, opting to give back instead of mire herself. She went to Belle Reve Memorial Hospital, starting as a candy striper there, and she did that well.

But while she worked at Belle Reve, she saw plenty of men who, in their immense stress, exhibited the same symptoms as Mr. Nas. When they took Mr. Nas off some of his medications, this is what resulted. She says that before Ben's next shift, she'll give him a Pepto and see how it goes.

According to the report, thirteen were killed in a bloody massacre at a Los Angeles clown school. According to the police, the attack occurred at 10:13pm (1:13am on the east coast), and the killer, Twisty the Clown, left behind a tarot card bearing the number thirteen.

Dressed in a sequin gown, Constance beckons her daughter, Adelaide, to the window to look at the moon. Of all the children under Constance's care, she has to admit, Addy was the most aware of something. What Addy feels in this moment is that the moon is powerful.
