Philadelphia is specifically mentioned by the doctor being interviewed on the talk show at the beginning of the movie. The location can further be inferred by listening to the place names mentioned by the characters in the helicopter as they fly away from the city.

Johnny and his sister Barbara were visiting their mother's grave when first attacked. Johnny fought the zombie and in the struggle was killed when his head struck a tombstone.

The black-and-white film coupled with the night-time fire lit lighting actually make the scene more terrifying, clearly showing organs and bits of flesh and yet leaving the types of organs somewhat to the imagination.

Dario Argento was the person who originally approached Romero with the idea for a sequel to "Night of the Living Dead", which had been a huge hit in Europe.
