On August 9th, 1992 in Montreal, Canada, James Hetfield was involved in a pyro accident. The set up of the stage had been changed from the previous show. But no one told James this, and at the start of "Fade To Black" he stood over one of the on-stage fireworks (not knowing it was there) as it went off, burning his arm and face.

The other band members rushed to help him and when James was taken to the nearest hospital, the other three band members re-took the stage to tell the crowd what had happened and that they were sorry but they couldn't continue the show.
